- Alcohol Permit (PDF)

- Occasionally serving alcohol at an event is allowed. If you are interested in applying for the application to serve alcoholic beverages, you will need to fill out the form and submit it at least fifteen business days in advance.
- Bake Sale Permit (PDF)
- For the protection of the student group and for those purchasing foods, the Bake Sale Permit must be completed and submitted along with a Sales and/or Fundraising permit.

- Food Permit (PDF)

- Whenever food is to be served at an event on-campus, a food permit must be completed and approved For all food policies, please visit the University of Minnesota Department of Environmental Health and Safety (DEHS) web site at www.dehs.umn.edu.
- Mailbox Rental Application Form (PDF)
- Completion of the Mailbox Rental Application Form ensures that your group's mailbox will be valid at 126 Coffman Memorial Union through the end of the current registration period. Student groups who have requested a mailbox in Coffman Union during registration must complete this form along with their other registration materials. Due to the limited number of mailboxes available, your request may be placed on a wait-list until a mailbox becomes available.
- Name Change (PDF)
- To change a student group's name requires the use of this special form (all other changes can be performed online by an officer). You must also submit a new, ratified constitution along with your Name Change Request Form to officially change your student group's name.
- Outdoor Space Application

- Are you interested in having your event outside? No matter what you want to do, whether it be tabling or having an outdoor concert you will need an outdoor space application, which starts the process to reserve the space for your event.
- Potluck Event Permit (PDF)
- Potluck events are defined as closed events where no money is exchanged and where homemade food is provided by group members to be consumed by group members.
- Request for Student Group Documents (PDF)
- To request copies of student group documents, such as constitution/by-laws, it is required that you fill out this form.
- Sales & Fundraising (PDF)
- Will your group be selling any items or fundraising as part of your event? If so, you must fill out this permit.
