- Equal Opportunity Statement
- All groups are required to sign the Equal Opportunity Statement each year, reflecting the University's commitment to equality.
- Event Planning
- Use this basic checklist to assist your group in planning a successful event.
- Food Permit
- Whenever food is to be served at an event on-campus, a food permit must be completed and approved. For all food policies, please visit the University of Minnesota Department of Environmental Health and Safety (DEHS) web site at www.dehs.umn.edu.
- Complaint Report
- Complaints against any student group can be registered with the Student Activies Office using this form.
- Name Change
- To change a student group's name requires the use of this special form (all other changes can be performed online by an officer).
- Outdoor Space Application
- All outdoor space reservations must have a completed Outdoor Space Application and meet with a Student Activities Office advisor to review event plans.
- Sales & Fundraising
- Groups performing any fundraising activities must complete this application first.
